COVID SCREENING QUESTIONS

Afternoon Lacrosse Families!

Everyone is thrilled to be hitting the floor with the SBLA this week!  With the return to the rink, we ask that ALL players MUST complete their 

• Attendance - Yes or NO 

and 

• COVID screening questions 

PRIOR to heading to the rink!  The availability can be completed, and changed anytime. The COVID questions can only be completed within 24hrs of the actual floor time. 

Please check your schedule for the yellow 'Health Check' box. If you do not see this box, and your floor time is within 24hrs, try refreshing or logging in and out of the app.
